First Visit
At East End Dental, we value our patients and strive to make everyone feel as welcome and comfortable as possible. If you are considering becoming a patient of ours, we have outlined some information you may find helpful prior to your first visit. We look forward to seeing you in the future!
What to Expect
Upon arriving, you will be asked to fill out a new patient form. One of our receptionists will gather your paperwork, insurance information, and a list of current medications.
Dental Examination
Your first visit to East End Dental will consist of a comprehensive exam and personal treatment plan. We will take the time to get to know you and what your goals are for your dental health. This will help us create a treatment plan that is tailored to your needs and expectations.
During your first appointment, we will review your medical and dental history. One of our assistants will take a series of digital X-rays, which allows the dentist to see anything that cannot be seen with the naked eye.
Forms
If you have already booked your appointment, you can download and fill out the forms below and bring them with you at your first visit.
Next, the dentist will complete a thorough examination of your teeth, gums, and surrounding structures. We review your treatment plan and go over any questions or concerns you may have.
Friendly Reminders
- Bring a list of current medications with you.
- Bring your insurance information and valid MCP card. We provide direct billing when applicable.
- If you are covered under Social Assistance, you will need your blue slip for the current month. You will also need a valid MCP card.
- There is a 24-hour notice requirement for all rescheduling and cancellations of appointments.
